How to manage Unilateral Hemiplegia & Generalised Seizure in kids? – Dr. Vykunta Raju K N
Unilateral hemiplegia in children are due to many reasons. Some of the reasons are it may be related to birth related problems, birth related injury of the brain at the time of the birth, in which the baby develops sudden onset of weakness of one side. Because of many reasons a clot in the arteries can be due to infection in the brain or due to bleeding in the brain or due to trauma during the birth passage time. So because of these problems the child can develop one side hemiplegia. Compared to the adult people the causes are various reasons. One may be due heart problem. If anything clot inside it can dislodge in brain and cause weakness on one side. Other causes may be related to blood related problems like lack of haemoglobin or blood calcium. Other cause may be brain related infections like tuberculosis or other infections like viral infections or brain fever or it may be related to the fall also. If any child comes with one side weakness generally we need to assess the child as immediate management. So we need to find out whether it is due to thrombosis or embolism or it is because of bleeding. So management differs in each case. So if it is due to bleeding, we need to find out the reason for bleed and we need to treat appropriately. If it is due to thrombosis or clot then you need to give medicines to dissolve the clot. Initially two medicines are there to dissolve the clot and most of the times they will come to the neurologist later, but at times we need to mange hemiplegia appropriately in the proper physiotherapy. If stiffness is too much then you need to give some medicines also. In physiotherapy for spasticity management sometime we require injection botulin toxin also. Regarding managing generalized seizures, usually generalized seizures commonly seen in other types of hemiplegia, quadriplegia. But hemiplegic also they can have convulsions. So there are various ways of treating generalized seizures in children. If only one type of seizure is there we will not consider medicine, if there are more than one attack we consider medicine. So based on child age or whether the child is a girl, other problems like liver or kidney problem we need to decide what medicines should be given. So we need to select medicines appropriately based on all these factors. Then you should be starting medicine and once you start medicine it should be given for 2 years till seizure free then it will decrease and stop. And if it is due to some other reasons if baby is not responding few more medicines need to be given, if not responding for a single medicine. Other option for generalized seizure in children is diet, other option is the surgery. These generalized seizure most of the times improve with medicines.
